Older Adult Allied Health Services
Constellation Therapies provides allied health services for older adults, with a strong focus on mobile, in-home support across Gympie and surrounding regions of Queensland. Our therapists come directly to the client’s home, supporting comfort, dignity and continuity of care.
We deliver a warm, respectful and professional service, taking the time to understand each person’s needs, preferences and goals. Our approach is person-centred, evidence-based and responsive to the realities of ageing, health changes and care environments.
Aged Care & Older Adult Services
We provide assessment and therapy services for older adults, including:
Clinical psychology assessments, including cognitive and psychological assessments
Counselling and psychological support
Occupational therapy to support daily living, safety and independence
Speech therapy for older adults, including communication, cognitive-communication and functional language support
Services are tailored to each individual’s functional needs and care context.
Swallowing Assessments and Dysphagia Management
We provide assessment and support for swallowing difficulties (dysphagia) in adults and older community members.
Our speech therapist has extensive experience in dysphagia management, including:
swallowing assessments
swallowing rehabilitation
mealtime assessments
development of individualised mealtime management plans
Swallowing support may be recommended for people who experience coughing or choking when eating or drinking, changes in voice quality, fatigue during meals, or ongoing chest infections.
We work with people with a range of presentations, including:
post-stroke
Parkinson’s disease
dementia and other progressive neurological conditions
Our approach focuses on safety, comfort, dignity and quality of life, with practical strategies that can be followed by individuals, families and support staff.

Mobile, In-Home Therapy
We prioritise mobile service delivery, providing therapy in the client’s home wherever possible. In-home sessions allow our therapists to assess functional needs in the person’s everyday environment and provide practical, meaningful support that translates directly into daily life.
Working With Families & Care Providers
We work collaboratively with older adults, families, carers and care providers, valuing clear communication and respectful partnership. Our clinicians provide practical recommendations, timely documentation and a consistent point of contact to support coordinated care.
Services are available through Aged Care funding, DVA, NDIS (where applicable) and private funding.
